Test–Retest Reliability of a Semi-Structured Interview to Aid in Pediatric Traumatic Brain Injury Diagnosis

Objective:
Retrospective self-report is typically used for diagnosing previous pediatric traumatic brain injury (TBI). A new semi-structured interview instrument (New Mexico Assessment of Pediatric TBI; NewMAP TBI) investigated test–retest reliability for TBI characteristics in both the TBI that qualified for study inclusion and for lifetime history of TBI.
Method:One-hundred and eight-four mTBI (aged 8–18), 156 matched healthy controls (HC), and their parents completed the NewMAP TBI within 11 days (subacute; SA) and 4 months (early chronic; EC) of injury, with a subset returning at 1 year (late chronic; LC).
Results:The test–retest reliability of common TBI characteristics [loss of consciousness (LOC), post-traumatic amnesia (PTA), retrograde amnesia, confusion/disorientation] and post-concussion symptoms (PCS) were examined across study visits. Aside from PTA, binary reporting (present/absent) for all TBI characteristics exhibited acceptable (≥0.60) test–retest reliability for both Qualifying and Remote TBIs across all three visits. In contrast, reliability for continuous data (exact duration) was generally unacceptable, with LOC and PCS meeting acceptable criteria at only half of the assessments. Transforming continuous self-report ratings into discrete categories based on injury severity resulted in acceptable reliability. Reliability was not strongly affected by the parent completing the NewMAP TBI.
Conclusions:Categorical reporting of TBI characteristics in children and adolescents can aid clinicians in retrospectively obtaining reliable estimates of TBI severity up to a year post-injury. However, test–retest reliability is strongly impacted by the initial data distribution, selected statistical methods, and potentially by patient difficulty in distinguishing among conceptually similar medical concepts (i.e., PTA vs. confusion).

The Rapid ASKAP Continuum Survey (RACS) is the first large-area survey to be conducted with the full 36-antenna Australian Square Kilometre Array Pathfinder (ASKAP) telescope. RACS will provide a shallow model of the ASKAP sky that will aid the calibration of future deep ASKAP surveys. RACS will cover the whole sky visible from the ASKAP site in Western Australia and will cover the full ASKAP band of 700–1800 MHz. The RACS images are generally deeper than the existing NRAO VLA Sky Survey and Sydney University Molonglo Sky Survey radio surveys and have better spatial resolution. All RACS survey products will be public, including radio images (with $\sim$ 15 arcsec resolution) and catalogues of about three million source components with spectral index and polarisation information. In this paper, we present a description of the RACS survey and the first data release of 903 images covering the sky south of declination $+41^\circ$ made over a 288-MHz band centred at 887.5 MHz.
Severity of Ongoing Post-Concussive Symptoms as a Predictor of Cognitive Performance Following a Pediatric Mild Traumatic Brain Injury

Objective:
This study aimed to examine the predictors of cognitive performance in patients with pediatric mild traumatic brain injury (pmTBI) and to determine whether group differences in cognitive performance on a computerized test battery could be observed between pmTBI patients and healthy controls (HC) in the sub-acute (SA) and the early chronic (EC) phases of injury.
Method:203 pmTBI patients recruited from emergency settings and 159 age- and sex-matched HC aged 8–18 rated their ongoing post-concussive symptoms (PCS) on the Post-Concussion Symptom Inventory and completed the Cogstate brief battery in the SA (1–11 days) phase of injury. A subset (156 pmTBI patients; 144 HC) completed testing in the EC (~4 months) phase.
Results:Within the SA phase, a group difference was only observed for the visual learning task (One-Card Learning), with pmTBI patients being less accurate relative to HC. Follow-up analyses indicated higher ongoing PCS and higher 5P clinical risk scores were significant predictors of lower One-Card Learning accuracy within SA phase, while premorbid variables (estimates of intellectual functioning, parental education, and presence of learning disabilities or attention-deficit/hyperactivity disorder) were not.
Conclusions:The absence of group differences at EC phase is supportive of cognitive recovery by 4 months post-injury. While the severity of ongoing PCS and the 5P score were better overall predictors of cognitive performance on the Cogstate at SA relative to premorbid variables, the full regression model explained only 4.1% of the variance, highlighting the need for future work on predictors of cognitive outcomes.

There is mounting evidence for the potential for the natural dietary antioxidant and anti-inflammatory amino acid l-Ergothioneine (ERGO) to prevent or mitigate chronic diseases of aging. This has led to the suggestion that it could be considered a ‘longevity vitamin.’ ERGO is produced in nature only by certain fungi and a few other microbes. Mushrooms are, by far, the leading dietary source of ERGO, but it is found in small amounts throughout the food chain, most likely due to soil-borne fungi passing it on to plants. Because some common agricultural practices can disrupt beneficial fungus–plant root relationships, ERGO levels in foods grown under those conditions could be compromised. Thus, research is needed to further analyse the role agricultural practices play in the availability of ERGO in the human diet and its potential to improve our long-term health.

Summary
To isolate an ideal form is to render it independent of the empirical domain and of noise. Noise is the empirical form of the message just as the empirical domain is the noise of form.

Iris Murdoch (2013 [1973]: 241)Interruptions, interceptions, flights, losses, holes, trapdoors … these figures of the motif of parasitism link the parasite to that property of communication referred to as noise. Following Michel Serres, we may acknowledge three broad domains in which parasitical arrangements operate: biology (tapeworms and so on), community (with, in certain communities, a special sacrificial role) and communication (noise, static, interference). As Serres observes, what was classically the biological sense involves a parasitical animal that ‘lives, eats, and multiplies within the body of its host’ (1980: 9). Yet this can seem to be a wide rubric: ‘We adore eating veal, lamb, beef, antelope, pheasant, or grouse, but we don't throw away their leftovers. We dress in leather and adorn ourselves with feathers’ (Serres 1980: 10). Serres's thought on the parasite, beginning with his multi-volume work Hermès(1968), and reaching a more sustained level in Le parasite(1980), helps to identify the paradoxical continuity of the parasitic structure across all its domains, thus transforming it in its concept. Any system whatever that involves as a condition of its operation the possibility of its interruption can be said to be essentially parasitic: ‘This system includes the telephone, the telegraph, television, the highway system, maritime pathways and shipping lanes, the orbits of satellites, the circulation of messages and of raw materials, of language and foodstuffs, money and philosophical theory’ (Serres 1980: 11). It is with this last category that Serres begins his investigation.
The first volume of Hermès, La communication, begins with a reading of the role of mathematics for Platonic dialogue, considered as an instance of the genesis of intersubjectivity and abstraction.

The commonest column colours of Actinia equina (Anthozoa: Cnidaria) are various shades of dark red or brown. However, these colour forms conceal genetically differentiated upper (U), mid (M) and lower (L) shore morphs. It is now evident that these morphs have differing ecological requirements and probably constitute different biological species. Identification by examination of diagnostic gene loci is not practicable for most ecological purposes. The L morphs can be readily identified, but there is no known morphological character to distinguish reliably between the U and M morphs. A study of acrorhagial characters indicates that, whilst neither the type, number, colour nor size of the acrorhagi themselves were diagnostic, there is a clear difference between the U and M morphs in the size of the holotrichs (a category of nematocyst). The holotrichs were significantly smaller in the U morph than in either of the other two morphs. In the M and L morphs the holotrichs did not differ significantly in size.

In the common intertidal sea anemone Actinia equina (Cnidaria: Actiniaria) the morphological plasticity and lack of hard parts has caused considerable debate as to what constitutes a species. Over the last several years genetic studies have indicated that this ‘species’ consists of various separate gene-pools which appear to function as cryptic biological species. Conventional taxonomy has often concentrated on the use of a cnidome, the type, structure and number of nematocysts ('stinging’ cells). However, the usefulness of nematocysts to distinguish among various morphs of A. equina and other anthozoan species has recently been questioned. Here we describe the first detailed study of nematocyst differences between two well characterized genetically differentiated morphs with different coloured pedal discs. Measurements were taken from each type of nematocyst in five different tissues. Contrary to expectations, clear and significant nematocyst differences were found between Actinia with red or pink pedal discs and others with green to grey pedal discs. These findings support previous electrophoretic studies and suggest that quantitative descriptions of the cnidome may accurately identify separate species within other genera of Anthozoa.

Summary
To interpret and use achievement test scores for cognitive diagnostic assessment, an explanation of student performance is required. If performance is to be explained, then reference must be made to its causes in terms of students' understanding. Cognitive models are suited, at least in part, to providing such explanations. In the broadest sense, cognitive models should explain achievement test performance by providing insight into whether it is students' understanding (or lack of it) or something else that is the primary cause of their performance. Nevertheless, cognitive models are, in principle, incomplete explanations of achievement test performance. In addition to cognitive models, normative models are required to distinguish achievement from lack of it.
The foregoing paragraph sets the stage for this chapter by making a series of claims for which we provide philosophical analysis and justification. First, we describe the philosophical standpoint from which the desire arises for explanations of student test performance in terms of causes. In doing this, we trace the long-held stance within the testing movement that is contrary to this desire and argue that it has serious weaknesses. Second, we address the difficult connection between understanding and causation. Understanding as a causal factor in human behavior presents a metaphysical puzzle: How is it possible for understanding to cause something else to occur? It is also a puzzle how understanding can be caused. We argue that understanding, indeed, can cause and be caused, although our analysis and argument are seriously compressed for this chapter.
